A Glimpse into the Science of Medical Archiving
Every organization utilizes a system of documentation in keeping a well-ordered record of their clients and employees. In medical centers, doctors and nurses use a system known as Medical Transcription. Medical Transcription is the science of archiving patient records and prescriptions in medical centers.
Medical Transcription, also called MT, is the system of transcribing medical reports into a text document. Health professionals commonly record findings and diagnoses of patients' health concerns through handwritten reports or voice-recorded files. During medical transcription, these are converted into computerized word documents which are kept in the archives of medical centers.
Individuals who transcribe medical reports are known as medical transcriptionists. Frequently, hospitals and private companies hire transcriptionists who have a degree in medicine and health sciences. They convert medical reports using databases and applications provided by the hospitals.
In the past, medical records are secured in a library of medical history. These are organized alphabetically based on the patient surnames. When doctors request for a patient's record, nurses delve into the archive shelves to find a patient's medical history. This old system of maintaining medical records cause inconveniences to doctors, nurses, and patients, giving way to the development of digital health records.
In modern medical transcription, doctors record diagnosis and analysis of patients' medical condition through voice recorders. This is known as Speech Recognition (SR) or Continuous Speech Recognition system (CSR). CSR and SR recorders are special kind of voice recorders which translates spoken words into written files. These provide a more convenient way forprivate and public keep medical reports as electronic health records.
There are instances when voice recorders misinterpret doctors' pronunciation of words and terms, resulting to improper encoding of medical terms. In order to maintain accuracy of information, medical transcriptionists edit the converted word files of SR and CSR. It is their responsibility to spot and correct mistakes in order to keepprecision in electronic health records of hospitals.
CSR and SR eliminate the need for manual search of medical records in hospital archives. Through this modern system, hospitals and medical centers find it more convenient to keep electronic health records of patients.
