The episode, like the show's pilot episode, was spread over the events taking place in the ER over a 24- hour period, starting and ending at 4 a.m. The events were the perfect blend of humor and gravity. The plot was brilliantly structured, which explains why millions of fans still download or watch ER online, to experience its magnificence. There are many starking similarities that one notices in the finale of ER and its early episodes.

An early scene of the finale mirrored the opening scene of the pilot, with the same nurse Lydia Wright waking up Dr. Archie Morris and Dr. Mark Greene in both episodes respectively. Furthermore, Dr. John carter demonstrated to Rachel Greene how to start an IV in the exact same way as Dr. Peter Benton demonstrated to him in the pilot. The scene was one of those brilliantly shot ones that still entice audiences to watch ER from their collections, or online.
