Diplopia Causes

When the visual axes of both the eyes are not in alignment

, the patient experiences double vision (diplopia). Study of the relative position of the images from the two eyes helps to determine the paretic muscle. The image arising from the normal eye is bright (true image) and that from the paralyzed eye is faint (false image). Ask the patient to look at the examiners finger which is moved in all directions.

Patients may experience cloudiness in the eyes. They may have difficulty focusing their vision that the objects they are looking may seem dull. Poor vision also prevents them from seeing the fine details. Moreover, patients suffering from blurring of vision may find in uncomfortable especially when they are in extreme emotional stress.

The cause of the problem is scarring in the brain stem which leads to a lack of co-ordination of the muscles controlling eye movement. In a healthy individual, eyes will both move in the same direction at the same time providing a single picture of the object they are focused on. If you have multiple sclerosis, double vision will be caused by your eyes moving independently of each other, causing them to focus differently and causing the visual problems.

Early symptom is headache, which is dull initially and it tends to worsen in the mornings. Later it becomes persistent and assumes a bursting character. Sharp rise in intracranial tension caused by coughing, straining at stools or stooping forwards gives rise to aggravation of the headache. Later the headache may become so severe and constant that the patient may not even complain of it unless specifically asked for.

Most of the patients are obese females who will complain of headache and visual disturbances. The headache is due to the raised intracranial pressure which is worse in the morning and aggravated by stooping or coughing or any action that raises the intracranial pressure like straining. The visual disturbances are in the form of papilloedema, secondary optic atrophy and diplopia due to the affection of sixth cranial nerve. The papilloedema may be severe and the visual fields will show enlargement of the blind spot.
