After the initial incisions, the skin is separated from the underlying fat and muscle tissues. The surgeon may use liposuction to trim excess fat in the face to give the patient a slimmer look. The surgeon will then tighten the most important layer of tissue the layer under the skin called the superficial musculoaponeurotic system (SMAS). Tightening this layer will help lift the cheeks, define the jaw and help firm the neck.

The procedure is finished by pulling the skin back over the area and trimming any excess. The incision is then closed with sutures, stitches, or tissue glue. These are generally placed in the area of the hairline so that they can be hidden by the patients hair. This process can take between two to four hours, depending on the complexities of the procedure.
