Passport Application Instructions for Changing Your Name

Whether you've gotten married, divorced or just decided that you were always really more of a "Summer Cloudburst," if you've changed your name legally you will need to change it on your passport as well as everywhere else.

The process for changing your name on your passport varies depending on how long you've had it. If you just your passport less than a year ago, here's what you'll need to do:

Complete passport application form DS-5504

Send the passport application form in along with your passport and legal documents to support the name change, such as a marriage certificate, divorce decree or court order.

Send in new passport photos as well, unless your passport was issued less than 3 months ago.

There is no fee to get your name changed on your passport as long as you are eligible to use form DS-5504

If you've had your passport for more than a year, here's what you'll need to do:

Fill out passport application form DS-82

Send it in with your current passport and legal documents to support the name change as per above.

Send in 2 passport photos

The cost is $110

If you were under the age of 16 when your passport was issued, you have another procedure to follow.

Fill out passport application form DS-11 in person at a passport acceptance facility

Bring your current passport.

Bring legal documents to support the name change and 2 new passport photos

Pay $135 if you are now over 16 and $105 if you are still younger than 16. Kids under 16 need both parents' permission to get a passport. See the Department of State's website for details.

Once you've submitted the paperwork, you should get a passport with your new name on it within the next 6 weeks if you applied for regular service and within the next 3 if you applied with the government's expedited service.
