Pepper spray is a derivative of hot cayenne peppers, there is a percentage of Capsaicin oil that's extracted from the stem of the hot pepper, which causes the pepper spray to stick to your skin and cannot simply be washed away which in return instantly causes unpleasant affects to your respiratory system and skin.

Pepper spray is an irritant and will cause difficulty breathing and affect the nose and throat, some reports indicate loss of muscle control, disoriented, confused and light headed. This may be due to limited oxygen, stress of being pepper sprayed or merely a side effect of the pepper spray.

There have been no reports of individuals passing away due to being sprayed by pepper spray. If you or someone you know has been sprayed and have any respiratory or heart problems it is recommended you see a doctor.
