Alzheimers disease (AD) was not the rare condition Alzheimer believed it to be, but rather, was the most common cause of senile dementia. Early-onset AD usually has a genetic element, whereby it is inherited in an autosomal dominant fashion. Genetic linkage analyses have led to the identification of four genes associated with a subset of cases of early-onset AD. An estimated 90% of AD cases are classified as being late-onset. Around 95% of the late-onset AD cases are sporadic in nature, lacking alterations in the known set of genetic markers, which suggests either environmental causes or a combination of environmental and predisposing genetic determinants. A considerable number of studies have been completed over the past 20 years to assess whether or not the presence of HSV-1 in the aged brain is associated with AD using increasingly more sensitive techniques. Although there are some contradictory findings, by and large, the studies indicate that HSV-1 is found commonly in the brains of individuals who had AD. With the advent of more sensitive polymerase chain reaction (PCR) techniques, the consistent detection of HSV-1 has indicated a strong correlation between HSV-1 in the brain and AD. The more recent PCR-based studies also indicated that HSV-1 is found at fairly high frequency in normal aged human brain, but is absent from most younger human brain samples examined. An absence of central nervous system (CNS) infection by a member of the herpes virus family was established, supporting the association of HSV-1 with AD. HSV-1, on its own, is not the sole causative agent because AD develops in a subset of those individuals with evidence of CNS infection but it is an established risk factor for AD. Similar to the association of HSV-1 and AD development, high frequency of recurrent HSV-1 disease is seen only in a subpopulation of HSV-1-infected humans. Most individuals are infected with HSV-1 early in life and this often results in only mild and self-limited or inapparent disease. Local replication seeds in innervating sensory nerve endings, which provides access to the peripheral nervous system (PNS), where HSV-1 establishes a life-long latent infection in sensory neurons, thereby escaping the host immune response and creating a reservoir for viral production. Reactivation leads to asymptomatic shedding or recurrent disease and the potential for the establishment of a latent infection in the CNS. It has been theorized that frequent HSV-1 reactivation in the CNS may lead to increased injury and neuronal loss and that this may predispose individuals to the development of AD.
