Psychological Injuries After Undiagnosed Tear

Perineal trauma during vaginal delivery

The Royal College of Gynaecologists and Obstetricians (RCOG) estimates that some 85% of women will suffer some form of perineal trauma during the course of a vaginal delivery. It is, therefore, part and parcel of a 'normal' birth. The severity of trauma will invariably differ from case to case, with injuries ranging from a first degree tear (a superficial injury to the skin) to a fourth degree tear (a serious injury extending to the sphincter.)

After the delivery, medical professionals must perform a thorough examination to ensure perineal trauma is identified. It is essential that an accurate diagnosis is made, as this will determine the course of treatment. For example, if a third or fourth degree tear has occurred, repair must be carried out in theatre by an experienced doctor. As long as a correct diagnosis is made within a timely fashion, there is a good chance that a full recovery will be made.


Physical and psychological injuries after undiagnosed tears

However, there are unfortunately times when a perineal tear goes undiagnosed. This may happen because:-

a) A vaginal and/or rectal examination is not performed after the delivery; or

b) The severity of the tear is not appreciated - eg. a second degree tear is diagnosed when in fact it is a third or fourth degree tear.

A tear that remains undiagnosed in the weeks, months or sometimes even years after the birth will cause a number of health complications for a woman, both physically and psychologically. The wound will remain painful and heal slowly, while additional symptoms such as urinary incontinence, faecal urgency and incontinence to faeces and flatus may also develop.

Evidently these physical symptoms will cause an enormous amount of distress for the woman concerned. Bowel dysfunction will be particularly troubling, especially if a woman has experienced episodes of soiling and incontinence. It may not be possible to resume employment and hobbies, while some will feel so embarrassed and fearful that they become withdrawn and socially isolated. Relationships with her partner may become strained as such symptoms usually lead to a degree of sexual dysfunction. Anxieties may also arise over the cosmetic defect itself, with many finding it difficult to come to terms with an altered body image. The combination of the physical, emotional and social factors will understandably cause significant psychological injury, with depression being a likely upshot.


Claiming for damages

If you have sustained a perineal tear that was not diagnosed soon after the birth, you have been the victim of a substandard level of medical care. This means you are entitled to claim compensation for your damages; not only does this cover the physical injuries, but also the psychological ones.
