The bible for ADHD testing is called the DSM (Diagnostic and Statistical Manual of Mental Disorders). The American Academy of Pediatrics advises practitioners to follow the guidelines set out in this manual for all pediatricians, psychologists and psychiatrists who may have to carry out ADHD tests. This sounds reassuring at first glance but the fact that there is no clinical test for ADHD and that this manual relies heavily on the use of questionnaires raises some doubts. At this point we have to face up to the disturbing truth that very often ADHD diagnosis is inaccurate or simply over diagnosed. ADHD testing starts with an initial physical and clinical examination to eliminate all the other possible conditions whose symptoms very often mimic those of ADHD. These consist of allergies, learning difficulties which have nothing to do with ADHD, allergies, food intolerances, hearing difficulties and even problems with eyesight. Then there is a neurological examination and then the questionnaires are filled out. Parents are asked about how often the symptoms of ADHD are noticed and usually asked for how long they have been present. These symptoms need to be present in all the settings, home, school and at play. Usually a period of six months or more is considered to be the minimum time and children under the age of four or five are not eligible for ADHD testing. In spite of that children of three years of age are diagnosed with ADHD and are put on psychostimulants! Then this is followed by other screening tests, questionnaires for the child's teacher, the TOVA (Test of Variables of Attention). Once all this information is gathered, the pediatrician or psychiatrist will refer to the DSM mentioned above and measure all the information according to the criteria set out there. Now these criteria are based on how normal children behave. All this seems relatively straightforward except that there is too much subjectivity in the whole process. As we are looking for a chemical imbalance in the child's developing brain, it is not so easy to seek a black and white answer. Once the ADHD testing is positive, parents have to look at the various options for treatment. They can choose between psychostimulants and behavior therapy or a combination of both. The whole area of treatment with mind altering drugs (with risks of addiction, side effects such as insomnia and loss of appetite) is a very dubious one and many parents are questioning this, much to the chagrin of the drug companies.
