Being disabled or having been diagnosed with a disease while working can limit the person in terms of his performance as an employee

The government agency would ask applicants to provide necessary information through submission and/or completion of different forms and documents which would later on determine the eligibility of the applicants for the disability program.

A person, for him to be allowed to receive benefits of the SSDI, must first file an application. An application form from the SSA is provided that needs to be completely filled-up with necessary information and then signed by the Social Security agency in California. After which, another set of documents and forms must be either submitted or completed in order for the SSA to further assess the eligibility of the person for the SSDI.

Speaking of documents, a work history report is to be provided by the applicant to the SSA. This would contain all possible work information, from the applicants employers, job descriptions, skills, to the requirements that come with each job in both physical and mental aspects. Also, the applicant must finish an Activities of Daily Living questionnaire. It contains questions that ask applicants about their day-to-day activities, like grooming, chores, and hobbies before the onset of the disability.
