That is simply because of the nature of the work they do. People need medical attention at all hours of the day, and if there is a need for an emergency medical delivery in the middle of the night it needs to be taken care of regardless of regular business hours, so these couriers exist to make sure that those deliveries can be completed. Most major centers now have at least one or two 24 hour couriers operating in their region.
