Handicapped bathrooms need to be not only more accessible, but safer as well. Building a personalized handicapped bathroom depends on the special needs of that physically challenged person. The quadriplegic will have a personal aid; the paraplegic will most likely be able to take care of himself.
The extreme elderly will also need an aid to help him. But, though the bathroom is handicapped equipped, it can still be slippery, so it has to be really specific. To do so, my top tips would be:

I had a case last week, a lady called up who was in her late 80s, and really should be living in an assisted living center. However, she said that she wanted to continue living in her own house, so we decided that we would try and do what she wanted. We could only build her a wet room as she wanted a shower she could walk into with her walker and also sit down in her chair, but that was impossible unless she had a wet room.
