When it comes to hemorrhoids, there is no one size fits all solution or miracle cure, treatment options depend on how serious the symptoms are which in turn is related to how advanced the hemorrhoid condition. Hemorrhoids are rated on a grading system from I-IV, with IV being the most serious grade. This grading system uses prolapse as the criteria for grading hemorrhoids, prolapse is simply when hemorrhoids become so swollen and distended that they can then fall out of the anal canal. Grade I hemorrhoids have no prolapse, Grade II hemorrhoids have prolapsed, but reduce on their own, Grade III hemorrhoids have prolapsed, but can be manually reinserted into the anal canal and Grade IV hemorrhoids are prolapsed but can no longer be manually reinserted.
The key to successful prevention and treatment is to treat hemorrhoids at the first sign of symptoms, but even if you have hemorrhoids it's not too late to potentially reverse the condition. Medically speaking, hemorrhoids can't be cured, but they can be reduced to the point that they no longer cause symptoms, so this is good enough to consider them cured. Common symptoms that can indicate a potential hemorrhoid problem include: anal bleeding, swelling around the anal region, itching and pain. It's possible to experience pain or discomfort while passing stools or just from everyday activities like sitting.
Prevention
Fiber is the key to hemorrhoid prevention. Everyone from the Food and Drug Administration (FDA) to the American Dietary Association (ADA) recognizes the importance of fiber, it's essential not just for promoting colon health and preventing hemorrhoids, but it's also essential in controlling blood sugar levels, reducing the risk of diabetes, helping with weight control, reducing the risk of diverticulitis, reducing the risk of heart disease and minimizing symptoms or preventing of irritable bowel syndrome. The benefits of fiber to overall health really can't be overstated.
The recommended daily allowance for adults is 25-35 grams of fiber, but most Americans only manage to get about 11-12 grams a day. This may help explain why the US has very high rates of obesity, diabetes, heart disease and hemorrhoids when compared to other industrialized countries, we're essentially killing ourselves with our diets.
Fiber works to help minimize constipation which is the leading cause of hemorrhoids. Constipation makes it difficult to pass stools - bowel movements become irregular and stools are hard and difficult to pass. All of this puts stress on the anal veins, which can then lead to hemorrhoids. Fiber helps to bulk up stools making them easier to pass, plus it works to make them softer which is gentler on the anal canal, both of these things help with both prevention and treatment.
People with Grade I and Grade II hemorrhoids may be able to completely reverse their symptoms with fiber alone, but once the condition becomes more advanced with worse symptoms or prolapse, it will not be enough to fully reverse the hemorrhoids. But, it's never too late to reap the benefits of fiber, because even if you do have a more advanced case, fiber can still help, less straining and softer stools can help to reduce swelling which then helps to minimize symptoms.
Treatment
Hemorrhoid treatment falls into one of two categories: 1) controlling symptoms or 2) removing the hemorrhoids. Controlling symptoms can be done with at home treatments such as using a sitz bath to soak the anal region, using ice packs or over the counter medications such as Preparation H or Tuck's Pads. If these at home methods don't work, then there are also prescription strength medications to control swelling and minimize symptoms.
If the symptoms can't be reduced or managed with these methods, then office procedures or surgery may be required to remove the hemorrhoids. There are a lot of excellent office procedures that can be done on an outpatient basis to remove hemorrhoids. The most commonly performed procedure is the rubber band ligation, but other outpatient procedures include coagulation which can be done with an infrared beam, a laser or with freezing, stapled hemorrhoidectomy, Doppler guided arterial ligation or sclerotherapy.
The option of last choice is the hemorrhoidectomy which involves removing the hemorrhoids with a scalpel, it is usually only used in the worst cases after other options have already been tried and proven unsuccessful.
Summary
As with all medical conditions, prevention is always preferable to treatment, this is why early intervention is so essential. If you start eating more fiber and take steps to improve your diet at the first sign of symptoms, then you may be able to completely eliminate your symptoms and reduce your risk of flare ups. Fortunately, for most people, hemorrhoids are a minor medical condition, but it can become serious enough to impact day to day activities and cause considerable pain, so to avoid potential complications such as prolapse, start hemroid treatment at the first sign of symptoms.
