Living donor should be evaluated by multidisciplinary team that includes a surgeon, transplant coordinator, psychiatrist, hepatologist, chest specialist, cardiologist, gynaec (in case of female donors) and authorisation committee to ensure that there are no elements of coercion. All necessary blood and radiological investigations should be done including blood group testing, LAI (Liver Attenuation Index), Hematology, biochemistry, serologies. Donors must have a compatible blood group with the recipient. Costly Imaging studies like triphasic CT and MRCP should be performed only after the basic evaluation completed and accepted. Advanced noninvasive methods like CT Scan computed tomography and MRI magnetic resonance imaging helps a lot in selecting a healthy and suitable living donor. In my experiences with about 600 living donor liver transplants invasive testing like liver biopsy should be performed at last if required in certain case like if LAI < 6 or BMI is more than 30.

A new Liver gives a new life and a well informed decision gives a successful Liver Transplant and complete satisfaction to the patient. The Patient and his family need to have proper Guidance and Education regarding Liver Disease and Liver Transplant procedure before going for this major undertaking. I want to help, guide and educate such patients through my experience as a Liver Transplant Coordinator and to share the moments faced by the patients in their Pre, Peri and Post transplant Period
