The primary question is whether the augmentation should take place above or below the pectoral muscle. Here's why the question matters.

Over the Muscle

Breast implants that are placed over the muscle are known as "overs" in the industry and the placement is called "sub-glandular", since the placement goes below the gland and over the muscle. One of the main advantages to using this type of surgery is the recovery time. Because there is no muscle manipulation, there is reduced pain and the patient will be able to recover much more quickly. Those with sufficient chest tissue may find that they get a more natural look by going over the muscle. It is also seen as the best choice for those with some degree of sag, if you don't want to go through a lift as well as augmentation.

Dual Placement and Under the Muscle

Partial sub-muscular placement refers to breast implants that go partially below the pectoral muscle. Complete sub-muscular placement refers to those that go completely underneath said muscle. This is often the ideal choice for women who don't have a lot of tissue in the chest or are looking to go larger than their frame would typically be able to handle. It will also help to reduce the "fake" appearance that can result from over the muscle placement in women who don't have sufficient chest tissue and skin. Women who have some existing sag can still choose under the muscle placement, but it may need to be accompanied by a lift.

Other Considerations

Of course, appearance isn't the only determining factor when deciding whether over the muscle or under the muscle placement is the best option for a patient. Surgeons must also take capsular contracture into consideration. Some studies have shown that going sub-muscular can help prevent contracture, though there is still a great deal of disagreement on the issue. Going sub-muscular also has the advantage of obscuring ripples in your breast implants.
