This is a paper that is going to give a literature review of the main topic which is Comparison of conservative treatment with and without manual physical therapy for patients with shoulder impingement syndrome: a prospective, randomized clinical trial. This study should give a clear understanding of why the study was done, how it was done, and the results and conclusions that were drawn.
Purpose of the study
The purpose of this study is to compare the effectiveness of two pairs of physical therapy treatment ways for impingement syndrome, either by a self-training program or by joint and soft tissue mobilization techniques.
Methods and scope of study
The study involved 30 patients who were grouped into two groups ,group one and group 2.group 1 patients of age 49.5 7.9 years were treated with strengthening the depressors of the humeral head with a girded self training program while group two patients of ages 48.17.5 years were treated by joint and soft tissue mobilization techniques.
Subjects
The subjects in the study were 30 patients of ages 39 years old to 58 years old.
Protocol and procedures
v Group one was instructed with the active range of motion (ROM), strengthening and stretching exercise program that included rotator cuff muscles, rhomboids, levator scapulae and, serratus anterior with an elastic band, for at least seven times a week for 10-15 minutes, while at home.
v Group two received a 12 sessions of joint and soft tissue mobilization technique, ice application, stretching and, strengthening exercise programs, with a 3 times per week patient education in the clinic.
v All patients were tested with visual analog scale (VAS) for pain level, ROM was measured by goniometric and pain threshold was measured by algometry.
v Three months after the initiation of the treatment, the function was measured with a functional assessment questionnaire, pain with functional activities was measured by vas and functional assessment questionnaire (NEER) was also measured.

Findings
Subjects in both groups experienced increases in shoulder function and a significant reduction in pain. Exercise group had no improvements that is 6.6 (1.4) to a post treatment mean of 3.0 (1.8) compared to the manual therapy group which showed significant improvements that is, pain was reduced from pre-treatment ( SD) of 6.7(0.3) to a post treatment mean of 2.0(2.0). ROM at flexion, abduction and external rotation in the exercise group did not show a significant improvement while in the manual therapy group a significant improvement was observed.
