"Volume loss" sounds like a mathematical concept, but in the context of aging, this means that you lose the softness and fullness in your face that you had when you were younger. Volume loss creates the dark shadows under your eyes that make you look tired and it causes the downturn of your lip corners (often called marionette lines) that make you look mad or sad even when you are not. Volume loss deflates your cheeks, and creates jowls. As aging continues, volume loss and damaged skin can culminate in a saggy, baggy face.

Traditionally, facelifts have been the main remedy for aging. But, facelifts alone can be limiting. Although very effective in lifting and tightening the face, there is no real replacement of lost volume. Consequently, after a facelift, your face may be tighter but not appear younger. Ideally, if we truly want to turn back the hands of time, we need a way to restore the loss of volume as well as tighten the saggy skin. The more modern approach to facial rejuvenation is to provide volume replacement with use of a filler that gives fullness and shape back to the aging face.

Presently, the choices for fillers include your own fat or one of a number of synthetic fillers. Synthetic fillers are made ready-to-use, in small aliquots, straight off the shelf, but they are temporary, lasting 8-12 months before being cleared away by your body. These man-made fillers are very useful if you have an isolated or smaller area requiring volume replacement, such as just marionette lines or nasolabial folds (the parentheses around your mouth), and especially if you want minimal downtime. It is possible to address multiple areas or even your whole face with synthetic fillers, however, the cost and issue of longevity makes volume replacement with fat as the filler more appealing when the whole face needs attention.


In fat augmentation or liposculpture, fat cells are taken from your lower body, usually the lower abdomen, inner or outer thighs, or waist roll. These carefully harvested fat cells are then gently processed and then used to sculpt the face, adding volume where it is needed. Typical areas on the face that are treated include the cheeks, under the lower eyelids, around the mouth, the jaw line to erase that jowl, and temple area. Using your own fat as the filler has many advantages: it is a completely natural product that is permanent, in relative abundant supply, that is easily harvested, which provides a soft fullness to the face. In addition, recent studies show that the fat appears to have special innate factors that rejuvenate the overlying skin. Because fat is living tissue, when the cells are taken from the "donor" site and placed in their new recipient location, not all cells will survive. So, it is possible that a touchup procedure may be required to provide the optimal outcome, but the cells that do survive are permanent.

Volume replacement with fat can be an excellent stand alone procedure in reversing the earlier changes of aging. If you already have more significant, later stages of aging, then fat augmentation in combination with a facelift can be transforming.
