Before the op

The operation
The technical term for the procedure is reduction mammaplasty, and it typically takes around 60-90 minutes, although more extensive surgery may take longer. The surgeon's goal is primarily to reduce breast size, and to flatten and enhance chest contours. You may also look at improving the size, position and shape of the areola, as well as removal of excess skin.
If the problem is primarily an excess of fatty tissue, the procedure may only involve liposuction techniques. A thin, hollow metal tube called a cannula is inserted through one or more small incisions, usually around the areola or in the underarm area, and moved backwards and forwards beneath the skin to loosen extra fat which is then removed by vacuum suction.
In instances where actual glandular breast tissue and excess skin have to be removed, excision techniques are employed, using a scalpel to cut away tissue, skin and fat. This is also the case if the nipple needs to be repositioned or the areola shape or size adjusted. The incision pattern varies in each case, but is usually made around the areola or in the underarm area.
In some situations a combination of liposuction and excision may be required.
The risks
As with any surgical operation, there are several potential risks involved in breast reduction procedures and you should speak to your surgeon about these before you commit to the surgery. They include:
* swelling, pain, scarring and bruising;
* infection;
* changes in nipple or breast sensitivity;
* irregularities in breast shape, contour and symmetry; and
* skin discolouration.
Recovery
You can expect to feel some pain, swelling, bruising and discomfort for a few days after the procedure, but nothing a decent prescription pain killer can't fix. You should be able to return to work as early as a day or two after the surgery and your stitches should be removed a week or two after it. To keep the incisions covered, the swelling down and the skin in place you'll probably need to wear an elastic pressure bandage continuously for a week or two and for a little longer at night.
You are probably going to be advised to avoid heavy exercise, strenuous work and sexual activity for a while, but should be back in normal working condition within about a month. The worst of the swelling will subside within the first week, but it may take three months or longer before the full results of the procedure will become completely apparent.
