There are a few things you need to do and few things you have to refrain from.
The first, you need to do on your own. Analyze your relationship, what went wrong, why the break-up and what was the root cause of it all. Think of what your girlfriend said at the time and use it as feedback to build the complete picture. In many cases, girls end up being taken for granted in a relationship and that is when they begin to lose interest in it. Girls loved to be loved and constantly need to be reminded of how special they are. Perhaps that is what went wrong with yours in the long term.
One thing you have to realize when you are trying to get your ex-girlfriend back is that the only things you can change are in relation to you. She is a completely different person and you cannot expect her to change the way she thinks. Start with yourself. If she agrees to see, give it some time first, then show her gradually how you have understood what went wrong and how you are planning on working on it. Observe her reactions and learn from it.
Also don't make yourself into a parasite of sorts. Give her space. It is difficult for her too and she will need time and space to heal and to be able to look at you in a different way. Don't seem needy and desperate by calling everyday, sending constant text messages and following her places, you will only drive her away. Make it a point to try and go on with your life. Show her that it is not the end of the world for you, but that it would be a better place if she were there with you. It takes time and you will need to work on it.
