Make a guitar shape out of card stock, and then use embroidery string for the strings of the guitar. If you really want to get fancy, add a custom guitar pick with the date and time of the event. Then hot glue the pick into the strings.

Record your birthday information onto a CD and then decorate the case. Maybe even put your favorite song into the background of the CD.
