Lore holds that there are other ways for wishes to be granted, but few of them are as charitable as the magic of a fallen star.

Whereas shooting stars are linked to positive change by divine actions, keys are connected to fulfillment by our own actions. Think about it and choose your tattoo design carefully! The short answer is that yes, getting inked can hurt. Overall though, it's not too bad. It's mostly the tiny repeated pricks on your skin and the tenderness afterwards. Tattooing in older times was much harsher and less safe.
