Mainly project manager is answerable for quality in a project. Still, it is the duty of each and everybody in the project to make sure that whatever thing they are doing to meet the project quality expectations. Project control-Repair defect and check whether the quality indicator are improving or not.
Good quality comes at cost. The better result of the product directly depend on the efforts are put. But continue investing for good quality is not at all a wise decision because there is no boundary to how good a product can be made. So, investment in quality is determined by understanding the highest level of quality. Highest level of quality is referring to that position where the incremental cost equals the incremental revenue.

Plan Quality:
Quality planning process identifies the appropriate standards necessary to execute the project efficiently to achieve the desired quality level. Outcome of quality planning is the creation of the quality management plan.
Perform quality assurance:
This determines whether the standards are being met, the work is continuously improving and deficiencies arebeing corrected or not.
Perform quality control:
Monitor and record result executing the quality activities to enhance the performance.
