The victim was removing the toggle plate seat from the pitman on a jaw crusher. He was standing on a temporary platform when the bolts holding the toggle seat were removed, causing the pitman to move and strike him.

Causal Factors

Management policies, standards, and controls were inadequate. Procedures were not in place to ensure that the pitman was blocked against hazardous motion before the toggle seat wedge bolts were removed.

Standards and controls were inadequate. No task training was provided to the miners regarding the removal of the toggle seat on the jaw crusher to make them aware of the health and safety aspects of the task.

Best Practices

1 Ensure that miners receive task training prior to allowing them to perform maintenance or repair on machinery or equipment.

2 Discuss the work procedures, including all possible hazards ( Risk Assessment ) and ensure the manufacturer's recommendations are followed for all repair work.

3 Before working on equipment, lock out the power and block equipment components against all possible motion/movement resulting from a sudden release of energy.

4 If potential hazards or prescribed procedures are unclear, DO NOT proceed until all safety concerns are adequately resolved.

Overview

The victim was fatally injured when he became entangled in a conveyor belt tail pulley at the portable crushing plant. He had entered the area under the crusher from the back end and traveled to the back side of the pulley. The accident occurred because management failed to establish

procedures to ensure that employees could safely remove spillage near moving conveyors. The conveyor was not de-energized and blocked against motion prior to persons entering the area. The victim failed to recognize the hazard of performing work near moving machine parts.

Root Cause

Operating procedures and controls were inadequate and failed to ensure that persons could safely remove spillage around moving conveyor belts.

Best Practices

1 Establish policies that ensure employees are trained on required procedures to safely remove spillage near moving conveyor belts.

2 Before assigning miners to clean up spillage, have them identify hazards and demonstrate how they would safely complete the tasks.

3 Stop, Look, Analyze, Manage ( SLAM )

4 Ensure that conveyor belts are de-energized and blocked against motion before positioning yourself near the drive, head, tail, and take-up pulleys.
