Rhinoplasty: Closed Or Open, Which Nose Surgery Is The Best?

Nose surgery is one of the most popular cosmetic procedures performed in the world

. Because it is so common, many patients are afraid of their surgery appearing too obvious and noticeable. Plastic surgeons have perfected two types of surgery techniques that leave minimal or no scarring, both of which when performed correctly are undetectable to the untrained eye.

Open Rhinoplasty

Many people shy away from this procedure because they are afraid the scarring will be more noticeable and people will be able to detect that they have had work done. While there will be a scar due to the incision, it is extremely small and will largely remain unnoticeable, unless perhaps someone is fixated on staring up your nostrils. The incision is created across the columella, which is the little bridge of skin that runs between the nostrils.

The incision allows the doctor to lift up the skin and reveal the muscle and cartilage beneath, which makes nasal reshaping more accurate and easier for the surgeon. There are some surgeons who use this method exclusively, while others may vary their techniques. However, in a few cases this is the only option possible, particularly when extensive and complex nasal tip reshaping is involved. A small scar is worth the more precise results.


Closed Rhinoplasty

Some patients will have the option of undergoing a closed surgery. The doctor uses internal incisions located inside of the nostrils in order to operate on the inside of the nose. It will be performed completely inside, which means there will be no visible scarring. Often times simpler rhinoplasties can be performed successfully using this technique.


Cartilage and bone can still be removed, added, or reshaped with a closed nose surgery. Bridge work, such as removal of a bump is generally possible; however, tip shaping can sometimes be limited as well as septum correction for breathing difficulties. Swelling and discomfort are sometimes reported as being milder than in open nose surgeries, but this is still largely dependent on the individual.

Both surgeries carry the same level and similar risks and complications including bleeding, infection, swelling, and bruising. They both have an overall longer recovery period than most cosmetic procedures. Nose operation takes up to an entire year to fully heal; however, results are visible immediately and will continue to improve in the following months. Initial recovery and discomfort only lasts one to two weeks.

Consult with a board-certified doctor to help you choose whether an open or closed rhinoplasty will best suit your needs. Revision nose surgery is almost always performed using the open technique due to the complexity and scar tissue. Although it is ultimately your decision which technique you'd prefer, it is best to seriously consider your surgeon's opinions (as well as consult with multiple surgeons before choosing one).
