The revised version is available on the USCIS website. More user-friendly features are incorporated into the revised version, which is dated 08/10/09. Applicants can either electronically file the Form I-90 or mail it to the USCIS Phoenix Lockbox facility.Generally, an individual can file Form I-90 to apply for replacement of permanent resident card. A conditional resident whose status is about to expire cannot use this application to request for replacement.
